This form is for your tax records as well as ours when making a donation to our school. Thank you for your generosity!

This is the form required in order to request a change in your student's pick-up or drop-off location. 

This form is only required of level 3 and level 4 volunteers. 

This form is required for level 1 and level 2 volutneers.

This form gives the school permission to post photos of your student on social media and in the press. 

Use of Electronic Media in the Knox County Schools constitutes the acceptance of these guidelines and the user’s assent to abide by the terms of use stated herein.

This form is optional; however, if it is filled out, signed, and notarized, it will allow your student to receive immediate medical care in the event of an emergency on a field trip. Highly recommended.
