Please print and return all pages of the Powell High Athletics Pre-Participation Packet to the Athletic Trainer or your head coach. 

 

Physicals for the 2025-26 school year must be dated after April 15, 2025 and all students are required to have a completed Pre-Participation Packet on file prior to participating in athletics at PHS.
