Need Learner's Permit form?

The Tennessee Department of Motor Vehicles requires a completed SF1010 when applying for a driving permit (This form is often referred to in schools as a Permit form). The state has set standards that each child must meet in order to qualify for the form.  These forms are completed by the school that your student is attending. 

Central High School follows the procedure below:

August-May: Student may pick up an application for the SF1010 in the main office It is completed and returned to the main office.  These forms are processed on Friday afternoons and will be ready for pick-up by 3:30.

June and July: Forms are attached below.  Completed forms can be dropped by the school's main office any Wednesday and picked up the following Wednesday.  The summer office hours are 9:00 AM-12:00 PM every Wednesday, except for the week of July 4th.
