Driver Permit Forms

If you need the S-1010 form as required by the DOT for a Learner's Permit to drive, then please call the school office at (865)689-9130 to make your request.  Please allow 5 business days to process your request.  The time may be longer during the summer, holidays, and other school closures.  The office staff needs this time to be able to manually process all of these requests.   Thank your for your patience and support.
