During the meeting I shared a Google Slides presentation; it had links to all of the "things" your child will need to bring to the first clinic day. Please use the checklist at the end to make sure all of the appropriate forms are completed. Here is the link to that presentation.
*A note about the KCS Physical Form:
If your child participated in a sport this school year (2020-2021), he or she does not need a new physical form in order to tryout. However, ALL dancers will need an up to date KCS Physical Form on record before they can participate in practices for the 2021-2022 school year; so in other words, by the end of school, all those who make the dance team, will need an up to date physical form - dated AFTER APRIL 15, 2021.

Please either sign, print, and return a copy to Coach Pace at Clinic Day on May 10 or 11 or scan, sign, and submit a copy to firstname.lastname@example.org.
